Patient Rights & Responsibilities

As a patient, you have the right to:

  • Expect to be treated with respect, consideration, and dignity
  • Assured confidential treatment of disclosure of records and opportunity to have voice in release
  • To know name and function of anyone providing care
  • To know names and relationships of other than own
  • To be provided known information concerning diagnosis treatment and prognosis
  • To have opportunity to participate in all aspects of your care
  • To request second opinion
  • To expect reasonable response to any reasonable requests you may make for service
  • To refuse treatment to extent permitted by law and be informed of medical consequences of actions
  • To expect communication in language which you understand
  • Expect treatment without regard to race, color, creed, religion, sex, national origin, or source of payment
  • To be aware of after hours or emergency care, educational materials, and payment policies
  • To examine and receive explanation of bill regardless of payment
  • To expect reasonable care time/location of appointments
  • To have smoke-free environment
  • To leave procedure area against medical advise
  • To have patient rights also apply to person who has legal responsibility for medical decisions on my behalf
  • To have pain assessed and treated
  • To have information concerning advance directives


Patient Responsibilities

As a patient, you are required to:

  • Provide complete and accurate information about your health, medications, including over the counter products
  • Follow the treatment plan prescribed by your provider
  • Provide a responsible adult to transport you home from this center
  • Accept personal financial responsibility for any charges not covered by insurance
  • Be respectful of all the healthcare providers, staff, and other patients
  • To make known to physician, nurse, center any concerns or complaints you may have
  • To make sure you understand all information given to you concerning your care and responsibilities associated with care
